Use this form to submit proof that you completed Title VI or sexual harassment prevention training this calendar year from a provider other than UB.
Please use this form only if you completed training outside of UB (for example, at a hospital or other work location). Individuals who complete UB's training course will automatically receive credit and do not need to submit a certificate.
In order to meet the UB's annual training requirement, you must have completed the training this calendar year. The training must also meet the following requirements consistent with outlined by the New York State Division of Human Rights.
You must review the university's policy prior to submitting your external certificate of completion.
New York State law requires all sexual harassment training to be interactive, meaning that employees must be afforded the opportunity to participate in the training. Training may be online, so long as it is interactive. Examples of employee participation include:
The definition of sexual harassment must be consistent with issued by the Department of Labor in consultation with the New York State Division of Human Rights.

This requirement would include an overview of laws that prohibit sexual harassment, such as Title VII of the Civil Rights Act of 1964 and the New York State Human Rights Law. It would also include information about the rights and protections of parties to a sexual harassment complaint, including a right against retaliation.

Supervisors have a heightened obligation to address sexual harassment they observe or that they hear about, and they are obligated to report harassment to Equity, Diversity and Inclusion.
